Denial Management explained
A denial is a payer's refusal to pay a claim it has already adjudicated. It is distinct from a rejection, which fails a format or eligibility edit before adjudication and can simply be corrected and resubmitted. The distinction decides the remedy: rejections are reworked, while denials are corrected and resubmitted or formally appealed inside a window set by the payer contract.
The common reasons are consistent from practice to practice: eligibility and coverage, missing or invalid prior authorization, coding and medical-necessity mismatches, missing information, duplicate submissions, and timely filing. Nearly all of them are decided before the claim is created, which is why denial work that stops at the appeal keeps producing the same denials the following month.
Effective denial management therefore has two halves. One recovers the individual claim. The other groups denials by reason code, payer, provider and procedure until the pattern behind them is visible, then fixes the upstream step producing it. Only the second half moves the denial rate.
If the same denials keep coming back, the work described here is our denial management service.

- Rejection
- A claim stopped by a format or eligibility edit before adjudication. It can be corrected and resubmitted as if new.
- Denial
- A claim adjudicated and refused. It needs correction and resubmission, or a formal appeal within the payer's window.
