Medical Coding explained
Medical coding translates a clinical encounter into the standardized codes payers adjudicate: CPT for the procedure or service, ICD-10 for the diagnosis that justifies it, and HCPCS Level II for supplies, drugs and equipment. The codes have to agree with each other and with the documentation in the record, because the payer reads them as one claim about what was done and why.
Coding errors cost money in two directions. Under-coding gives away revenue the practice has already earned, and it is invisible because the claim still pays. Over-coding creates compliance and audit exposure. Between them sit specificity and modifier problems, which mostly cost time — a medical-necessity or bundling denial, a rework cycle, and an appeal window shorter than the filing window was.
The discipline that prevents all three is the same: code to the documentation, and query the provider when the note does not support the code. It is slower on the day and considerably faster across the month.
If coding accuracy is the constraint rather than claim volume, look at our medical coding services.
Terms that keep coming up
- CPT
- Current Procedural Terminology — the code set describing the procedure or service that was performed.
- ICD-10
- The diagnosis code set describing why the service was performed. It has to support the CPT code on the same claim.
- Modifier
- A two-character addition that qualifies a procedure code — bilateral, distinct, repeat, professional or technical component — without changing it.
