Revenue Cycle Management explained

Revenue cycle management is the financial process surrounding medical billing, from the moment an appointment is scheduled to the moment the last dollar for that visit is collected. Billing is the claim-focused middle of it; RCM adds registration, eligibility and benefits verification, payment posting, denial work, patient balances, and the reporting that shows whether any of it is working.

Practices lose revenue at predictable points in that cycle. The front end loses it to registration and verification errors that are cheap to prevent and expensive to appeal. The middle loses it to coding specificity and filing deadlines. The back end loses it to unposted remittances, unworked denials, and balances that age quietly past the point where a payer will reconsider them.

Because the stages are connected, the useful diagnosis usually sits upstream of the symptom. A rising days-in-A/R figure is often a first-pass acceptance problem from two months ago, and a denial rate that will not fall is usually a registration or authorization problem rather than an appeals problem.

To see how these stages are sequenced for a practice that hands the cycle over, read our five-step billing process.

Terms that keep coming up

Front end
Everything before the claim exists: scheduling, registration, eligibility and benefits verification, and prior authorization.
Back end
Everything after submission: payment posting, denial work, A/R follow-up, patient balances and reporting.
Net collection rate
Payments received as a percentage of what payer contracts actually allowed, after contractual adjustments — how much of the collectible revenue was captured.
