Medical Billing explained

Medical billing is the process of turning a patient encounter into a paid claim — capturing the patient's details and coverage, coding what was done, submitting the claim to the payer, posting the remittance, and pursuing whatever comes back unpaid or short. It is a sequence rather than a task, and every stage inherits the accuracy of the one before it.

That sequence is why a billing problem is rarely where it first appears. A member ID mistyped at the front desk surfaces six weeks later as an eligibility denial. A missing modifier surfaces as a partial payment nobody reconciles against the fee schedule. The further downstream an error is caught, the more it costs to fix and the more likely it is to have already repeated.

For a practice, the questions worth asking are narrow: how many claims are accepted on first submission, how long money sits in accounts receivable, what share of denials were preventable, and whether anyone checks payments against the contracted rate. Those four numbers describe a billing operation more honestly than any description of it.

If you would rather hand the cycle over than run it in-house, start with the eight billing stages we manage end to end.

Terms that keep coming up

Clean claim
A claim the payer accepts on first submission, with no rejection, edit or request for further information.
First-pass acceptance rate
The share of claims accepted on first submission — the earliest signal that registration, verification and coding are correct.
Explanation of benefits
The payer's line-by-line statement of what was allowed, paid, adjusted or refused on a claim, with a reason code for each.
